Using our Online Patient Portal – ConnectMed (secure online site)

Patient portals are secure online sites, provided by GPs, where patients can access their health information and interact with their general practice. They give people convenient and secure electronic access to their health information, increasing their ability to manage their own health.

Register for the patient portal by completing the ConnectMed Enrolment Form which can be obtained from reception or by clicking on this link:

A patient portal allows enrolled patients to go online and:
Important Points:
  • Each family member must have their own unique email address.
  • Those over 16 years of age may need to provide photo identification for copying.

On receipt of your completed ConnectMed Enrolment Form we will enter your details and you will then receive an email from ConnectMed that must be responded to within seven(7) days. That email will contain a link to the ConnectMed Patient Portal where you must twice enter a password you have chosen.

Please check your junk mail folder for the email if it hasn’t come through after a couple of days, and please contact our Reception team if not found.

Once enrolled with ConnectMed please keep these points in mind:
  • When requesting a repeat prescription that you intend for us to fax to a pharmacy, you must enter the pharmacy’s fax number or it will not come through. Pharmacy fax numbers are usually available from their web pages.
  • After requesting a repeat prescription the amount owed will be displayed along with our bank account number should you wish to pay online.
  • You will only see lab results from the day that you are registered with the Connectmed Patient Portal.
  • You can use ConnectMed on your mobile as the ConnectMed website is compatible with mobile device browsers. The website will automatically fit your device.
